AC-CE-0002-MB-23 RFSQ for Laser Equipment, CO2 and Holmium

This Request for Supplier Qualifications (“RFSQ”) is an invitation by the Coordinating Entity to prospective respondents to qualify in accordance with Evaluation of Responses (Part 2) for eligibility to provide Laser Equipment, CO2 for ENT & Gynecology and Laser, Holmium for Urology as further described in Section A of the RFSQ Particulars (Appendix B) (the “Deliverables”).
The Coordinating Entity, as identified on the cover page, is issuing this RFSQ and administering the RFSQ process on behalf of the Purchaser(s) identified in Section B – Material Disclosures of the RFSQ Particulars (Appendix B).
Two (2) categories have been identified in the RFSQ for the Laser, CO2.  Evaluation of the submission will take place on a category-by-category basis as follows:
Category 1 - Laser, CO2 for ENT
Category 2 - Laser, CO2 for Gynecology
Five (5) categories have been identified in the RFSQ for the Laser, Holmium.  Evaluation of the submission will take place on a category-by-category basis as follows:
Category 3 - Laser, Holmium, Urology - Maximum energy delivered must be 60W or less
Category 4 - Laser, Holmium, Urology - Maximum energy delivered must be between 61 to 99W
Category 5 - Laser, Holmium, Urology - Maximum energy delivered must be between 100W to 119W
Category 6 - Laser, Holmium, Urology - Maximum energy delivered must be between 120W to 149W
Category 7 - Laser, Holmium, Urology - Maximum energy delivered must be 150W or greater

This RFSQ will identify the minimum clinical requirements for respondents to be selected per category and considered pre-qualified to move forward to negotiated Form of Agreements to be included on a roster for as needed Equipment purchases.  Scores per category from this RFSQ will move forward to a secondary request for quote (RFQ) process requesting discount off Equipment and consumables from the roster participants on an as needed basis.  The highest combined scores per category from RFSQ and RFQs will be awarded the Equipment purchase.  The Equipment purchase may be awarded by the Coordinating Entity as a whole, or by individual sites requirements per category.  The Equipment may be required by one or all of the Purchasers as will be defined in the applicable RFQs.  As such, RFQs may be coordinated by any of the Purchasers.
