HOUSTON - Primary Care Clinic Renovation RFP

The Houston diagnostic and treatment (HDT) offers episodic and longitudinal primary and community care, short-term urgent and emergency care, and limited specialized services, including community paramedic services, while also supporting clinical pathways and linkages to higher levels of care.

Increased service volumes into the Primary Care Clinic (PCC) (and other programs) at HDT has led to significant space and patient flow issues. There is an urgent need to renovate and expand the current Houston Primary care clinic within the Houston Health Centre.

The schematic renovation plan developed to expand footprint and make changes to existing footprint to add additional Primary Care Clinic (PCC) area:
1. Resize both washrooms to accommodate spatial requirements for; 4 new Exam Rooms (3 adjacent, and 1 opposite).
2. 2 new Exam/Office Rooms for Inter-professional Team (IPT) members.
3. New Waiting Room for 5 people.
4. New Reception with transaction window opening into Waiting Room, Desk, and passthrough unit for specimen collection.
5. New shared Office big enough for two Primary Care providers.
6. Separate entrance as shown in schematic.

This is a working site and to accomplish the above requires the designer to provide and design for temporary PCC space for; 3 Exam Rooms and 2 Exam/Office Rooms and provide the phasing to accomplish this. See Appendix B for an example of what this phasing might look like.
